Lidocaine and Prilocaine Gel

Lidocaine and prilocaine gel is a topical local anesthetic that contains a mixture of two numbing medications. It is designed to be applied to the skin or mucous membranes to cause numbness or a loss of sensation before certain medical procedures.

Lidocaine and prilocaine are both local anesthetics from the amide class. When combined, they form a "eutectic mixture," which means they have a lower melting point than either substance alone. This allows them to be formulated into a cream or gel with a higher concentration, enabling them to penetrate the skin more effectively.

Their action involves blocking nerve signals in the treated area. They do this by inhibiting the conduction of pain signals from the nerves to the brain, which provides temporary numbness and pain relief.

Lidocaine and prilocaine gel or cream is used for a variety of purposes, including:

• Dermal Anesthesia: To numb the skin before procedures like:

  • Needle insertions (e.g., injections, blood draws, IV lines)

     

     

  • Minor skin surgeries (e.g., wart removal)

     

     

  • Laser hair removal and tattooing

     

     

• Genital Anesthesia: To numb the genital area before minor medical procedures.

   

   

• Dental Procedures: A specific periodontal gel (e.g., Oraqix) is used to numb the gums during dental work.

   

   

• Premature Ejaculation (PE): It is sometimes used as an off-label treatment or in specific over-the-counter products to reduce penile sensitivity and help with premature ejaculation.

   

   

The gel is applied in a thick layer to the targeted area and is often covered with an occlusive dressing (a special bandage) to keep it in place and help with absorption. The length of time it needs to be on the skin (usually 1 to 2 hours) depends on the procedure and the patient's individual needs.

Dosage varies by patient and procedure and should be used exactly as directed by a healthcare professional. Applying too much or using it over a large or broken skin area can lead to unwanted systemic effects as the medicine is absorbed into the bloodstream.

Lidocaine and prilocaine gel is generally safe when used correctly, but it can cause side effects and has important warnings.

Common Side Effects:

• Redness, paleness, or swelling at the application site.

   

   

• Itching, rash, or a mild burning sensation.

   

   

• Changes in temperature sensations.

   

   

Serious Side Effects (Requires Immediate Medical Attention):

• Methemoglobinemia: A rare but serious blood disorder where red blood cells are unable to carry oxygen effectively. Symptoms include pale, gray, or blue-colored skin; headache; rapid heart rate; shortness of breath; and fatigue. • Allergic Reactions: Symptoms like hives, difficulty breathing, or swelling of the face, lips, or tongue.

   

   

• Systemic Toxicity: If too much of the gel is absorbed, it can cause severe side effects such as dizziness, drowsiness, blurred vision, ringing in the ears, seizures, or an irregular heartbeat.
